How to access your medical records.

Lab results​:

  • Once your lab results have been processed by the front office, they will be made available in your Patient Portal. If you do not have a Patient Portal account, contact our team at 478-216-5176. An email address is required for a Patient Portal account.

 

Sending records to your doctor:

  • We can send your medical records directly to your chosen provider's office (or any third party of your choosing). To provide permission for those records to be sent, please complete an Authorization to Release Medical Records. Text our team at 478-216-5176 to receive the form. This form can also be completed in-person at any of our three clinics. Photo identification is required.

  • Your doctor's office will also have an Authorization to Release Medical Records form available. If you have already completed one there (or their office is more convenient), it can be faxed to our Privacy Officer at 478-755-1346.
